Lessons Learned One-Pager Creator
Turn a closed HSF-002 incident investigation into a single-page, tri-branded awareness handout for toolbox talks and notice boards. Fields mirror the investigation report so you can copy across without re-thinking the story. Everything happens in this browser — nothing is uploaded.

No names — by design
A lessons-learned sheet gets photocopied, posted and emailed onward, so it must never identify the injured person. This tool does not collect an employee name at all: the HSF-002 name, job title, body part, injury nature, medical provider, W/C claim and investigator fields are deliberately absent and are never carried over. Write about people by role — "the operator", "the associate", "the driver".
